Private Fireworks Shoot Permit

  

 REQUIRED DOCUMENTS

Copy of vendor pyrotechnic license

Copy of public display permit from State Fire Marshal's Office

Certificate of insurance for $1,000,000 bodily injury and $1,000,000 property damage, naming the City of Wolfforth as co-insured

Site plan for shoot, including dimensions

Inventory list of fireworks

Written permission from property owner (if not on applicant's property)

Proof of ownership (if on applicant's property)

Proof of supervision arrangements with Wolfforth PD

Deposit cash, cashier's check, or money order with Wolfforth PD for supervision fee

Proof of standby fire truck arrangements with Wolfforth Fire/EMS

Deposit cash, cashier's check, or money order with Wolfforth Fire/EMS for standby fee

CLEANUP REQUIREMENTS

Remove all debris in shoot field and within two city block radius fall zone, within 24 hours of completed

shoot. Director of Public Works will inspect. Cleanup deposit will not be refunded if inspection fails.

$50.00 Application Fee

$500.00 Cleanup Deposit

$100.00 Police Supervision (per officer)

$325.00 Fire Truck standby with 3 man crew
